Service Description
***CONTRACTED SERVICES*** Basic W-B Assessment: Intake Questionnaire + Up to 2 Hrs. Assessment/Observation + Bullet style report + Travel up to 35 minutes *Add-Ons highly recommended. Standard W-B Assessment: Basic W-B + Report Writing + Travel up to 1.5 Hrs. *Add-Ons highly recommended. Comprehensive W-B Assessment: Basic W-B + Report Writing + 1 Needs Assessment + Travel up to 1.5 Hrs. *Add-Ons recommended if additional assessments are needed.
Cancellation Policy
No-shows or failures to cancel appointments, sessions, or plans at least seventy-two (72) hours will result in a non-refundable session fee and/or administrative fee if applicable. Failure to reschedule within at least forty-eight (48) hours in advance will result in a non-refundable session fee and/or administrative fee if applicable if client is a no-show. Services and/or plans purchased by a third-party sponsor or donor and not the individual/client will not render a refund directly to the individual/client. We will also reserve the right to deduct any service fees according to our cancellation policy and services rendered.
